Requesting Sacramental Records

Sacramental information is always recorded at the church where the sacrament was performed.  To request certificates from an open Diocesan Parish, please contact the parish directly.

Closed Parishes

Once a parish is no longer open, all sacramental registers are transferred to the Diocesan Archives & Records Office and are under the authority of the Chancellor, as per the Diocesan Sacramental Records Ownership and Access Policy. We continue to care for these records like a parish would, updating sacramental notations and issuing certificates.

The following is a list of sacramental registers held by the Diocesan Archives & Records Management Office:

  • Bishop's Confirmation Register, 1947-1969 (includes confirmations performed on GA military bases)
  • Camp Villa Marie, Savannah
  • Camp Wheeler, Macon
  • St. Patrick's Parish, Augusta
  • St. Patrick's Parish, Savannah
  • Immaculate Conception Parish, Augusta
  • Most Pure Heart of Mary Parish/St. Mary's Parish, Savannah
  • Nativity of Our Lord Parish, Savannah
  • Sacred Heart Parish, Augusta
  • St. Stanislaus Chapel, Vineville/Macon, 1888-1912
  • Willacoochie Missions, 1923-1937

Requesting Sacramental Certificates

Please complete a Sacramental Certificate Request Form and mail it to us with a photocopy of a valid id (driver's license, passport, etc.). There is no administrative fee for the request.

You may only request your own certificate unless you are a legal guardian or have power of attorney.
The Archives & Records Department will not honor any requests for sacramental certificates from third parties other than parishes or Diocesan officials. This includes parents, spouses, friends, etc.

Records for Military Personnel

If you or a family member were active military when you received any sacraments on a US military base in South or Middle Georgia (such as Ft. Benning, Columbus; Ft. Gordon, Augusta; Ft. Stewart/Hunter Army Airfield, Hinesville/Savannah; Moody Air Force Base, Valdosta; Robins Air Force Base, Warner Robins; US Marine Logistics Supply Base, Albany; and Naval Submarine Base, Kings Bay) those records are kept by the Archdiocese for Military Services. Records Older than 85 Years

Information from sacraments celebrated and recorded more than eighty-five (85) years ago is available upon request without discrimination. A register falling within this time frame is accessible to the public if it is digitized, microfilmed, or a preservation copy. We currently have a selection of sacramental registers (see above) that have been digitized and are fully available to the public to view from our Reading Room. Original Sacramental Registers are not open for public examination in order to protect the original books from wear and ensure the privacy of records that are less than eighty-five (85) years old. Research in original books must be conducted by authorized Diocesan personnel.

Record Searches

A Sacramental Information Request Form must be completed for sacramental record searches conducted by the Diocese of Savannah Archives & Records Management Office. Because staff is limited, no sacramental information will be accepted over the phone or via e-mail and requests will take a minimum of 4-6 weeks. All requests must be accompanied with valid proof of identity (a photo id with a signature) from the person requesting those records. A $25 administrative fee will be required for each sacrament that is requested. This is a non-refundable administrative fee and will still be charged in the event that no results are obtained. Fees may be waived at the discretion of the Director of Archives & Records or Chancellor.

Because of the way the sacraments are recorded, we search for information by individual and sacrament. We are happy to provide this information upon request; however, we do not perform family tree or full genealogical line research.
