- Complete application
- A personal handwritten statement from the wife of a married applicant indicating her support of her husband’s application into aspirant formation
- Certified birth certificate
- Copy of social security card
- Copy of passport and green card (if applicable)
- Copy of recent (within 3 months) medical examination certificate
- Four (4) letters of recommendation
- Signed consent form for criminal and general background checks under the auspices of the Office of the Permanent Diaconate
- Signed Final Statement of Veracity and Release of Information
Sacramental Documents:
- Certified copy of your baptismal certificate with notations within the past 6 months
- Copy of your confirmation certificate
- Marriage certificate (if applicable)
- Divorce and/or certificate (if applicable) and Decree of Annulment
Academic Documents:
- Official copies of all high school, college, graduate school or any other educational institution transcripts to be sent to Rev. Mr. Kelley G. Culver, Director (address above)
- A written report of a rector of any previous house in which the applicant has spent time in formation including explicit reference to the evaluations of the candidate and the votes he received
Photographs:
- Two recent passport-size photos of you and your wife (if applicable)
Detailed autobiographical sketch (please type or print) to include the following:
- Chronological history of your life
- A brief description of your relationship with your parents
- Relationships with family members and other significant people
- Experiences and achievements in school and work
- Social life
- Relationship with God, prayer, and the Church
- One significant success you have experienced in your life
Two short essays on the following topics (please type or print):
- “What does the Diaconate Mean to Me”
And
- “What is My Understanding of Living a Celibate Life if I am Single Now (if applicable)
Or
- What Does My Marriage and Family Life Mean to Me? (if applicable)
